The Help Me Grow Care Coordinator provides care coordination functions, which assist families and providers to understand the services available through early childhood development and behavioral networks including, the early intervention system, early special education programs, special health care needs programs, and makes referrals to these programs as appropriate. The care coordinator handles calls related to the health, development, behavior and learning of children, and supports families by providing information and education on these topics, researching and making referrals to services, and providing continued support, advocacy and follow up as needed. Care Coordinator handles telephone cases, documents case information, mails information packets, and networks with staff and all community-based providers.

Responsibilities      

  • Assesses caller/client situation and enters accurate data on all calls into the HMGA Family Database and makes referrals to appropriate services as needed
  • Provides care coordination for families, researches most appropriate resources, provides education and information on development, behavior management and services, and provides advocacy and follow-up as needed
  • Assists in identifying gaps and barriers to services, and system issue trends families experience in utilizing services
  • Maintains HMGA database system for all cases, community resources, and pediatric and family medicine practices in the specified region and ensures information is accurate and up-to-date for required reporting
  • Supports medical practices and early childhood programs in fully developing protocols, screening, and connections to HMGA in the specified region and utilizing HMGA services through information, education, parent outreach, and community outreach
  • Administers the Ages and Stages Questionnaires® (ASQ-3 and ASQ:SE-2) to interested families which includes, but is not limited to, distributing and scoring, providing families with developmental activities, addressing any care giver concerns, following up with any necessary and appropriate referrals, and managing the data entry for all completed questionnaires into the ASQ Online Database
  • Ensures the confidentiality of all data from program participants, in alignment with specific requirements and protocols for database data entry, data analysis and reporting, project evaluation, and information sharing
  • Ensures accurate and up-to-date reporting on all outreach associated with HMGA in specified region
  • Assumes additional responsibilities and performs special projects as needed or directed
  • Conduct outreach and recruitment of physicians
  • Conduct Help Me Grow educational presentations
  • Collaborate with statewide Help Me Grow partners
  • Participation in Community Initiatives’ Continuous Quality Improvement Plan
  • Other duties as assigned

Qualifications

Knowledge & Skills

Knowledge and experience in early child development

Experience in working with community resources and systems

Interpersonal skills and ability to maintain a calm and sensitive demeanor

Excellent time management and organizational skills

Excellent written and oral communication

Comfortable and effective public speaking

Ability to work in a teamwork environment, as well as independently

Computer proficiency utilizing Microsoft programs, Internet, and database management

 

Education/Experience

Bachelor’s degree in child development, social work, public health, or related human services field preferred.

At least 1 year work experience in child development, social work, public health, or related human services field.

Working Conditions

Normal office environment.

Routine travel to early learning centers and other places as needed.

Flexible schedule with some afterhours and/or weekend work.

Some out- of- town travel required.

Lifting up to 30 lbs.

 

Requirements

Valid Alabama driver’s license

Reliable transportation

Proof of automobile insurance commensurate with State law

Background checks to include but not limited to criminal record and drug screen

Must meet and maintain eligibility requirements to be insured by UWCA’s insurance
