- Organize outreach events to include marketing and planning related activities to assist in promoting the Family Stability Services program. Perform a variety of outreach and recruitment activities through direct interaction with residents and community stakeholders
- Manages assigned caseload from Family Stability referrals and provides quality case management and housing counseling services, including development of budgets and financial plans for all participants in the Family Stability program.
- Conduct assessments to include at a minimum housing, money management and educational attainment
- Develop plans to help families achieve housing and financial stability and help children maintain school enrollment
- Obtain the family’s endorsement of and participation in the plan along with requisite releases, authorizations to provide outreach, advocacy and/or case management
- Provide progressive case management and monitor outcomes with family until family achieves case plan objectives and for a minimum of three months
- Connect families with needed and available community resources
- Follow-up with clients and community resources as appropriate to document use/success of referral
- Adheres to program guidelines and prescribed service delivery as identified by funding sources
- Maintain client records and input ongoing client notes into program’s case management software, including intake documents, releases and ongoing progress towards goals. Maintains accurate records in accordance with professional, agency and applicable grant standards.
- Refer clients to financial education classes. Develops and/or facilitates client family night workshops.
- Participate in regular staff meetings, training programs, and supervisory sessions
- Adhere to agency policies, procedures and the professional code of ethics
- Participate in Community Initiatives’ Continuous Quality Improvement Plan

Ability to establish rapport
Education/Experience
Bachelor’s degree in SW, Counseling, or social service-related degree
Experience in counseling/case managing high risk populations
Excellent computer skills to include Microsoft and database
Working Conditions:
Normal office environment
Occasional work in the community – including travel to and from sites
Some after-hours and/or weekend work may be required
Occasional out of town travel
Requirements:
Must meet and maintain eligibility requirements to be insured by UWCA’s insurance
Valid Alabama driver’s license
Reliable transportation
Proof of automobile insurance commensurate with State law
Background checks to include but not limited to criminal record, drug screen
